Working Papers Information

If you reside in the Hastings-on-Hudson School District or attend private or parochial school, you must bring all of the following documents to the HASTINGS HIGH SCHOOL COUNSELING OFFICE in order to receive your working papers:

• Complete the Application of Employment Certificate and the Physician's Form.
  (Download the forms HERE.)

• Your doctor must complete the physical form, sign it, and stamp it. 
  You must have had a physical within the last year.

• Evidence of Age: birth certificate or a passport showing date of birth is acceptable. 
  Hastings students do not need to present evidence of age; their schooling record will suffice as identification.

All of the above items must be presented at the time the actual working papers are issued.
